A range of evidence from population-based studies provides a robust framework on which to build a life course model of schizophrenia and related psychoses. Understanding the biology of the close association between psychotic illness and development undoubtedly holds keys to the epigenetic puzzle that is schizophrenia. Adopting a life course approach to schizophrenia, the literature is reviewed regarding putative mechanisms and early life risk factors as manifest from conception to onset of illness. Environmental, developmental, and cognitive domains are included, and evidence is highlighted from population-based studies over the past quarter century.
