A 54-year-old man attended the emergency department with a five-day history of fevers and night sweats. There were no other symptoms. He had received a pre-emptive live donor renal transplant from his wife four months earlier upon reaching end-stage renal failure due to IgA nephropathy. The patient’s medications included: tacrolimus 4 mg two times daily (bd), mycophenolate mofetil 500 mg three times a day (tds), prednisolone 7.5 mg once a day (od), and nifedipine M/R 10 mg bd.
