A 62-year-old woman attended the haematology clinic for assessment of a monoclonal protein of 12g/L. Examination showed no lymphadenopathy or hepatosplenomegaly but assessment of blood pressure showed a postural drop and peripheral oedema was noted. She had a mild normochromic normocytic anaemia.
